There are different grades of neuropathy in the diabetic person. Diabetic neuropathy might be a focal syndrome, a distal sensorimotor and/or autonomic syndrome or it might be subclinical and simply show up as an abnormality on neurologic testing. 

Diabetic neuropathy is a major public health problem and should be treated as soon as possible.

How Alpha-Lipoic Acid Helps With Diabetic Neuropathy

DIabetic neuropathy is the end-result of chronic low-grade inflammation and oxidative stress. The hyperglycemia and poor blood flow found in diabetics start the problems. 

There is an accumulation of advanced glycation end products (AGE); these bind to receptors on the neurons themselves and cause oxidative stress.

There is poor circulation at the microscopic level that feeds the nerves themselves, this results in hypoxia and nitric oxide inactivation. The AGEs also induce NF-kB (nuclear factor-kappa beta) which further causes oxidative stress.

The hyperglycemia and hypoxia also damage the mitochondria of the nerve cells over time. The oxidation causes lipid peroxidation and cellular destruction.

The increased levels of glucose in the serum of diabetics leads to auto-oxidation and the production of AGEs. This is considered a major cause of the ROS found in the peripheral nerves and other organ tissues

This glycation damages natural antioxidants and therefore, not only are more ROS produced but fewer can be cleared. 

Exogenous (external) sources of antioxidants are often needed to keep these free radicals in check.

Benefit #1: Healthy Aging

Calorie restriction causes what is known as ‘healthy aging’. This means that there is an improvement in the metabolism of a cell. 

In addition, there is the creation of new and healthy mitochondria to deal with this calorie restriction. This is a normal survival mechanism of humans.

By restricting caloric intake for longer than the normal physiologic periods of fasting (8-14 hours), you can limit the development of age-associated damage to the metabolic abilities of the cell. This calorie restriction also causes a reduction in cellular aging.

Basically, a high fat diet will cause an increase in body weight and an increase in blood glucose levels. There is a reduced ability of cells to have insulin-mediated glucose uptake. The glucose transporter Glut-4 is down-regulated in as soon as a month of a HFD.

Glut-4 is very much reduced in skeletal muscle, for instance the calf muscle (gastrocnemius). In fact, fast-twitch muscles like the calf muscle, which is essential to mobility, are the most affected by a HFD along with aging and the most damaged.

 

Atgl is protective for cells exposed to diets high in fat. A high fat diet will actually reduce the basal cell metabolic rate. This reduces overall oxygen consumption. As well, the length of mitochondrial DNA decreases in the face of a diet high in fat and this is one of the primary markers of aging. Thus, a diet high in fat will accelerate aging.

The normal effects of aging and DM2/obesity on skeletal muscle is atrophy and dysfunction. The muscle develops insulin resistance and there is wasting and weakness.

Persistent nutrient overload results in major changes to cellular metabolism and thus early aging. Too much nutrition also generally results in more ROS production and more oxidative stress damage to the cells. Time-controlled fasting (or intermittent fasting) can stop this.

Healthy muscle cells can be flexible to the nutritional needs of a person. These mitochondria can switch between the oxidation of fatty acids and glucose pretty quickly. Fasting can enhance evolutionary conserved mechanisms of longevity.

There are many things that can impact your mitochondrial function – and one of the main factors is what you choose to eat.

What Are Mitochondria, And How Are They Related To Aging?

The main purpose of the mitochondria is to produce energy for the cell from dietary inputs. This is done via what’s known as ‘cellular respiration’. Aging reduces this ability of the mitochondria. Reactive oxygen species (ROS,) or free radicals, are a normal byproduct of energy production.

However, these are very damaging molecules if not managed properly. As we age, the amount of ROS exceeds the cell’s management ability – and oxidative stress and cell damage results.

How Aging Affects The Body

Some tissues are very energy intensive. One of these tissues is skeletal muscle. Aging then, is particularly harmful to the function, form and abilities of skeletal muscle. 

This is because the mitochondria become dysfunctional and there are far too many oxidants around and way too few antioxidants.

Similar to the effects of aging are the effects of a high fat diet. Therefore, if you eat a HFD and you are aging, you might be in trouble. Being sedentary and eating a HFD is causative of mitochondrial exhaustion. This creates massive damage to your body and brain. This is true of your muscle as well.

Obesity and Type 2 Diabetes actually cause a slow-down in cellular respiratory function (energy production) and will increase the mitochondrial ROS production. 

This causes an increase in loss of functional muscle. This also enhances all effects of aging.

AMPK and TOR are major upstream regulators of mitochondrial metabolism. These molecules are essential with regard to how dietary restriction induces longevity and weight loss. 

TOR (target of rapamycin) is activated by certain amino acids such as leucine. This gene induces protein synthesis and is stimulated by energy availability.

When TOR is activated, it causes a reduction of autophagy/mitophagy – the clean up of damaged mitochondria. Insulin and high levels of glucose also stimulate the release of TOR; TOR basically stops the maintenance of good and functional mitochondria. This is pro-aging.

 

AMPK as stated is very important to healthspan. This regulator senses stress and energy levels; it responds accordingly. If in a low nutrient situation it will activate metabolic processes and produce ATP. 

It will inhibit biosynthesis and control the change from catabolism to anabolism. AMPK is activated in low energy states; when energy stores are depleted. This is sensed through tiny increases in the AMP/ADP:ATP ratios.

When AMPK is turned on, it restores cellular energy by inhibiting energy-intensive process such as protein synthesis, glycogen synthesis and fatty acid synthesis. 

Also, energy-producing reactions are stimulated. These include glucose uptake, glycolysis and fatty acid B-oxidation. In multiple species, activation of AMPK is known to extend life and improve health.

Activated AMPK also inhibits TOR. Inhibition of TOR will likewise activate AMPK. AMPK activation always increase autophagy/mitophagy and promotes cellular cleanup of damaged structures. 

AMPK also participates in a positive feedback loop with SIRT1 and SIRT3. THis is through an increase in the NAD+:NADH ratio and sirtuin activation. 

This of course results in deacetylation of mitochondrial proteins involved in metabolism and lifespan.

How Intermittent Fasting Can Help Reverse Aging

Calorie restriction is essentially a 20-40% reduction in caloric intake, but without malnutrition. This slows aging and extends a healthy lifespan.

The most simplistic way to consider GABA is to know that it is inhibitory of neuronal activity.

By inhibiting neural activity, GABA reduces physical and mental stress, calms anxiety, and therefore helps induce a calm sleeping state. GABA plays a large role in the balance of inhibitory and excitatory signals for the brain.

This study found that participants with 30% lower GABA levels in the brain suffered from primary insomnia. Studies support the notion that human stress and anxiety disorders have GABA impairments as a foundational problem.

Many heavy pharmaceuticals target the GABA receptor. For example, benzodiazepines, picrotoxin and barbiturates target the GABA-A receptor. Benzodiazepines are anticonvulsant, anxiolytic and sedative-hypnotic. They increase the frequency of ion channel opening in response to their action on the receptors. Phenobarbitol has been used for over 100 years to treat epilepsy; it too is GABAergic. Baclofen targets the GABA-B receptor.

What Is CoQ10?

Coenzyme Q10 (CoQ10) is a naturally occurring, fat-soluble antioxidant used for cell growth and support. It is an essential nutrient for the health of all human tissues and organs [1]. 

The enzyme is found throughout the body, especially in the inner membrane system of the mitochondria and is a fundamental part of the body’s ATP cycle: ATP is adenosine triphosphate and this is the form of energy used by cells. 

Major organs such as the heart, lungs, liver, kidneys, spleen, pancreas, and adrenal glands are rich in CoQ10. Although the body synthesizes CoQ10, components such as nutritional deficiencies, genetic defects, illness, and aging can significantly decrease CoQ10 levels.

Without the powerful antioxidant capacity of CoQ10, free radicals formed in the cell are able to damage DNA, proteins, and lipids: resulting in oxidative stress. Antioxidants, such as CoQ10, are enzymatic and nonenzymatic agents that are able to prevent the formation of or completely remove free radicals after they are formed.

 It is well-known today that CoQ10 is one of the most effective lipid antioxidants that is able to prevent the generation of free radicals and modifications of proteins, lipids, and DNA [1]. In addition to reducing and neutralizing free radicals, CoQ10 regenerates local antioxidant agents such as tocopherol (Vitamin E). 

Individuals with conditions such as heart disease, diabetes, migraines, and statin-induced myopathy will benefit significantly by incorporating a CoQ10 supplement into their diet. 

CoQ10 is found in many foods, such as oily fish, meats, whole grains, and nuts.

CoQ10: The Super Molecule

The body is unable to synthesize enough CoQ10 for optimal health. Extra amounts must be ingested through food or supplementation. 

CoQ10 helps to increase the number of lymphocytes (white blood cells to fight diseases and infection) as well as improve endothelial function by increasing nitric oxide release from endothelial cells which contributes to overall cardiovascular health. 

The immune system and physical performance also relies on the use of CoQ10. The tissues and cells that are a part of the immune system are energy dependent and require an adequate supply of CoQ10 for optimal function [1].

CoQ10 offers protection from statin-induced myopathy.

Cardiovascular disease, such as atherosclerosis, ischemic heart attacks, stroke and other maladies are the number one health problem in the modern world. Generally, first-line treatment of high cholesterol in western countries is a prescription of statins. Statins have been very successful in the treatment of high levels of cholesterol. 

Cholesterol has been directly linked to cardiovascular disease. Statins work by reducing serum low-density cholesterol levels (LDL). These act to stop the synthesis of cholesterol and to also increase the hepatic low-density lipoprotein receptor activity. But, they also reduce levels of CoQ10 in the human body.

The best way to reduce the risk of cardiovascular disease is to consume a healthy diet. However, many physicians assume patients will be non-compliant with dietary changes, resulting in medications being prescribed that bear serious side effects. 

For statins, these include severe damage to cellular mitochondrial function along with muscle pain, fatigue, weakness, and cramping. 5 to 10% of people taking statins to control cholesterol will have such side-effects. 

CoQ10 is a key substrate for energy metabolism and essential for cell membrane stability. The cell membrane is where many cellular functions occur. In the face of CoQ10 deficiencies, muscle cells are prone to damage, disease, and in the worst case scenario, total destruction.

The risk for statin induced myopathy can be reduced through orally supplementing coenzyme Q10, which has been demonstrated to be a safe intervention to help with muscle pain. 

CoQ10 is depleted by statins and replacement is necessary. Statins are one of the most commonly prescribed drugs in the world. The statin drugs include Crestor (rosuvastatin), atrovastatin, fluvastatin and simvastatin among others. Statins exert their effect on cholesterol by reducing the activity of a key enzyme (HMG-CoA reductase). 

However, anything downstream of this enzyme is also reduced. CoQ10, a ubiquinone antioxidant, is downstream of this enzyme’s activity. Thus, not only is LDL reduced, but so is CoQ10. Muscle cells are prone to damage in the face of a CoQ10 deficiency; this results in myositis or myopathy. 

For two groups of people on statin with statin-induced muscle pain, the group that took CoQ10 had a significant reduction in muscle pain. As well, muscle fatigue, cramping, weakness and tiredness also decreased. The dosing recommended is usually 150mg to 200mg each day.

Ideal For Fighting Migraine Headaches

According to the World Health Organization, migraines are the third most prevalent and sixth most disabling disease in the world. Migraines are a product of a chronic neurovascular disorder: disorders within the blood vessels that carry oxygenated blood to the brain and spinal cord. They are characterized by severe, throbbing unilateral headache attacks with nausea, vomiting, and photophobia. 

The malfunction that takes place within nerve cells when a migraine occurs can be attributed to oxidative stress, inflammation, and impaired oxygen metabolisms: all of which occur due to mitochondrial dysfunction. CoQ10 is an essential cofactor for proper mitochondrial function. 

Treatment of migraine begins with avoiding so-called ‘triggers’. For example, some foods that ‘trigger’ migraines include chocolate, cheese and monosodium glutamate (MSG). Physical activity helps to reduce the frequency and intensity of the pain. Proper sleep also helps a great deal. Climate issues matter in terms of the local temperature for a person. Many people with migraines bring personal fans wherever they go to manage their micro-climate.

This is one of the factors that cause what we term “aging.” What we commonly call ‘aging’ is really just the end-result of long-term accumulation of free radical damage and low-grade chronic inflammation. Persistent and chronic inflammation will deplete our natural production of CoQ10. 

Certain drugs that are common in older persons will as well. Such drugs include statins which are prescribed to lower levels of cholesterol.

Damage to the cells from free radicals is another main component of aging. CoQ10 is one of the most powerful antioxidants and it can regenerate our own antioxidants and others. It is able to effectively integrate into the cell membrane for protection. 

About 80% of our normal use of oxygen happens in the mitochondria. Here, oxygen is used with food molecules to create usable fuel, or energy, for cellular functions. Spinal region pain, particularly pain in the lower back, is extremely common in the United States and in countries around the world. Most adults have had low-back pain at some point in their lives, and back pain is one of the most common causes of missed work days, temporary disability, etc.

Most low-back pain resolves itself naturally within a short period of time. This kind of pain, which lasts for up to 4 weeks, is known as acute back pain. Back pain that lasts beyond 4 weeks is called subacute. After 12 weeks, it’s classified as chronic low-back pain. Although this seems arbitrary, it is a good way to think about these sorts of pain.

Chronic low-back pain can usually be resolved with non-surgical and non-narcotic methods like brace, yoga, acupuncture, NSAIDs, acetaminophen and/or physical therapy, but sometimes, it persists. Today, we’re looking at some natural ways to relieve low-back pain in the comfort of your own home.

CHECK YOUR VITAMIN INTAKE

Studies that have included blood panels on those with low back pain have consistently found that those with chronic low-back pain are often deficient in vitamin D. 

“Most patients (83%) attending spinal and internal medicine clinics in Saudi Arabia over six years who had experienced low back pain that had no obvious cause for more than six months had an abnormally low level of vitamin D.”

But before you get a stand-alone Vitamin D supplement, it’s important to make sure you’re taking the right amount - and in the right ratio to other supplements. Magnesium absorption, for example, is far more efficient when taken with vitamin D3. It is very hard to reach levels of Vitamin D3 that are toxic. There is no consensus on the best levels of D3. There are simply minimum levels we know will help people avoid rickets. But for pain-free living in an optimal health range, the level of serum D has not been fully established.

For better absorption of vitamin D, you must include vitamins like magnesium and zinc in your diet, or take them as an additional supplement.

Most authors in the field of spinal issues would agree that only about 1% of cases of lower back pain actually ever need surgery. Sometimes, inflamed muscle groups (“trigger points”) can pull on the connective tissue and muscle supporting your spine, causing pain. Dry needling is an effective way to release these knotted muscles and reduce pain.

Sometimes, pain or dysfunction in other areas can contribute to back pain. If you’re walking with knee or foot pain, this can offset your balance and posture, pulling on your back and causing pain. 

There are deep and superficial lines of connected fascia that run from the foot to the skull; tightness or mobility impairment anywhere along these fascial lines can lead to lower back pain. A skilled physician or therapist (massage or physical) should be able to identify these issues.

This is why it’s so important to stretch daily and especially before exercise, to make sure you aren’t suffering from joint pain that can lead to misalignment or a dysfunctional mobility impairment chain. It’s also essential to have a good muscle recovery plan in place after activity.

If your back pain is mild, you can also try exercises, like certain yoga stretches (cat-cow, child’s pose, downward-facing dog) to alleviate soreness! Walking has a lot of good data behind it to support this low-impact aerobic activity for the treatment of common back pain.
