Common Interactions With Well Theory Supplements

About ¼ of Americans that take prescription pharmaceuticals also take supplements. Somewhere between 40% and 60% of those with chronic diseases do as well. Side-effects of natural products are exceedingly rare, however many people wonder if the natural product will interfere with the prescription medication.

How Our BODIES Process MEDICATIONS AND HERBAL SUPPLEMENTS

Prescriptions are generally processed in the liver in humans. Certain enzymes within the liver are responsible for breaking apart the drug and then allowing the biologic effects to take place.  

Interactions between an herb or extract and a drug usually occur as a pharmacokinetic interaction; this means the processing rates of each are changed. 

If the rate of processing in the liver changes, then the level of the drug and its effectiveness could also be changed. 

In order to change the pharmacokinetics of a given pharmaceutical, the herb must change the absorption, distribution, metabolism and excretion of the drug.

How Pharmacodynamics Are Changed

Occasionally, an herb or extract might directly interfere with the way a drug works. This is called a change in the pharmacodynamics.

Most of the drug-processing in the liver uses enzymes in the cytochrome P450 super family. In fact, about 80% of all manufactured drugs use these enzymes. 

There are 6 that are considered common. These are the following:

  • CYP1A2
  • CYP2C9
  • CYP2C19
  • CYP2D6
  • CYP2E1
  • CYP3A4

Of these, CYP2D6 and CYP3A4 are responsible for almost 50% of all drugs. If you know what enzyme is primarily utilized by your medication, then you can assess the herbal interaction with that given enzyme – if they use the same enzyme.

Drug Interactions

with Anticoagulants

Of all medications, people most commonly ask about the interaction of herbs and extracts with anticoagulants.

Warfarin

Warfarin is still the most commonly prescribed anticoagulant globally. This is a Vitamin K antagonist. For those on Warfarin, a very tight range of serum levels are needed for effectiveness. The CYP2C9 enzyme is heavily involved in the metabolism of Warfarin. Most interactions with Warfarin involve other prescription drugs, or OTC drugs.

Some herbal remedies are known to act on the CYP2C9 system in the lab, but clinically there is no issue. No food or nutrient has been shown to strongly interact with Warfarin (unless it contains Vitamin K). Soybeans hold a lot of vitamin K. There is a potential interaction with grapefruit due to the furanocoumarins.

There is one reported case of an interaction with a woman on Warfarin who consumed large amounts of chamomile too; this was due to CYP1A2, 3A4 and maybe 2C9. There is a clinically significant reaction of mango and Warfarin due to the inhibition of CYP2C19 by retinol found in the mango.

St. John’s wort will reduce the effectiveness of Warfarin.

Thrombin And Factor Xa Inhibitors

Direct oral anticoagulants are another class of medications to thin the blood. These include thrombin and Factor Xa inhibitors. 

There is no direct evidence of food or herbal interaction here. However, it is wise to discuss herbs such as St. John’s wort with your physician of course. 

Most direct factor Xa inhibitors are processed by the CYP3A4 enzyme.  Some supplements increase levels of medication in the blood, while others will reduce the levels. The same is true for direct thrombin inhibitors.   

Herbs Known To React With Medications

Two herbs are known to have clinically relevant interactions with prescription and OTC medications; these are St John’s Wort and Goldenseal.

There is one study that showed an effect of curcumin on CYP1A2 (it induced enzyme activity). This could make it interact with antidepressants or antipsychotics. It may also interact with the 3A4 enzyme.

Most physicians become concerned when a patient is on chemotherapeutics; in this case, as always, you should check with the prescribing physician.

Both cinnamon and turmeric are thought to increase anticoagulation a bit as well. Many people on Warfarin avoid turmeric due to the increase in bleeding potential.

Of course, many people on anticoagulants still take aspirin or NSAIDs daily. These definitely cause problems with prescription anticoagulants; these problems are more clinically relevant than any due to a plant or herb.

Herbal Supplements Compared To

The risks inherent to supplements are the same risks inherent to eating or drinking the same plants.

For example, there is a case report of a man on dabigatran who was hospitalized for bleeding after consuming a mixture of boiled cinnamon and ginger.  In fact, herbal teas are the most commonly consumed supplement by those on such anticoagulants.

If you are on a direct thrombin or Factor Xa inhibitor ask your physician about the following herbs and see what dose they might recommend: danshen, dong quai, ginseng, ginger, gingko biloba, green and chamomile tease, St. John’s wort, turmeric.  

Do Fruit And Berry Supplements

Brightly colored fruits and berries have abundant amounts of phytochemicals and antioxidant power.

For example, tart cherry extract holds anthocyanidins and these allow the extract to provide excellent pain relief and strong antioxidant benefits.

In general, the effect of plant phytochemicals such as anthocyanidins, proanthocyanidins and phenolic acids is much weaker on liver enzymes than the effect of grapefruit (furanocoumarins).  Berries and berry extracts are poor inhibitors of liver enzymes. They have little to no effect on most drugs.

Consult With Your Treating Physician

For surgery that requires anesthesia, one rule of thumb is to stop herbal remedies about 5 days prior to the procedure. This will ensure there are no interactions with the agents used for sleep and pain control during surgery.

If you have a concern, we always recommend that you consult the treating physician. However, most American-trained physicians will likely have no idea how to answer the question.

Not many people are familiar with GABA. So what is it? GABA or Gamma-aminobutyric acid is a neurotransmitter that helps send messages between the brain and the nervous system.  This nonprotein amino acid is found in almost every prokaryotic (lacks organelles or internal membranes) and eukaryotic (having a nucleus and internal membranes) organism.  It is a large part of the pool of free amino acids actually.  

Gaba is a bioactive compound that may be found in foods. In plants, it acts in the Krebs cycle and helps with energy production. For humans, it is a powerful neural signal transmitter. Gaba has been located within tomatoes, tea, soybeans, brown rice (germinated) and others. Fermented foods are a particularly good source of this vital amino acid product. Even sourdough bread is a great source of GABA.

In mammals, GABA should be found in high concentrations in the brain.  In fact, somewhere between 60% and 75% of the brain synapses are GABAergic.  

Our bodies naturally produce in our brains to reduce nerve cell activity. So why should you be taking GABA daily?

Improves Sleep

GABA has a calming effect and can naturally help induce sleep by making your nerves less excitable.

One study found that when participants were given GABA, they were able to fall asleep faster. Those with insomnia are likely to produce GABA naturally in lower levels.

GABA helps to protect the neurons from cell death.  This will assist the brain during sleep’s detoxification and clean-up functions.  

One of the side-effects of drugs such as gabapentin is sleepiness.  Gabapentin acts on one of the GABAergic receptors.  Both baclofen and gabapentin have demonstrated the ability to protect neurons from cell death as well.  

Multiple studies have looked at GABA with regard to sleep improvement and the results are very promising.

Reduce Anxiety

Since GABA mainly works to reduce nerve excitability, it has been linked to reducing the feelings of anxiety and fear. Some anxiety disorders have been associated with low levels of GABA.  GABA is effectively a way to tamp down the excitable effects of a different neurotransmitter, glutamate.  GABA has been used to treat problems such as depression and sleeplessness.  Obviously, lack of sleep would enhance the detrimental symptoms of depression and anxiety. 

A study done in the American Journal of Psychiatry found impaired GABA responses in participants with panic disorder, an anxiety disorder defined by recurring panic attacks.

Benzodiazepines, such as Xanax or Valium, strongly bind a certain GABA receptor actually.  The threshold for seizure, anxiety, panic and stress response are all regulated by certain GABA receptors.  Other GABA receptors manage mood, pain and memory.  Scientists believe that most of the effects of GABA on the brain actually happen in the peripheral nervous system.    

High Blood Pressure

GABA supplements are sometimes used as a natural method to lower blood pressure. Poor sleep and sleeping disorders have been known to contribute to higher blood pressure and hypertension.

In one study, participants with borderline high blood pressure were given a chlorella supplement high in GABA. The results of the study showed a significant reduction in blood pressure.

Animal studies have also shown that ingestion of GAGA-rich foods will decrease blood pressure.  GABA-enriched fermented soybean diets lowered blood pressure.  As well GAGA-rich green tea also reduced blood pressure in hypertensive rats.  Similar findings have been seen in human studies as well.  An intervention with fermented milk, holding between 10 to 12mg of GABA, reduced BP in hypertensive adults.  Another study showed that supplementation of just 80mg of GABA reduced BP in adults with (mild) hypertension.  

Reduce Inflammation

Research has found that GABA has the potential to decrease inflammatory responses. GABA is believed to reduce activity in the nerve pathways that trigger joint inflammation.

One study on mice monitored the supplementation of GABA found that the risk of developing arthritis was reduced and symptoms in those that did develop arthritis were less severe.

GABAergic circuit alterations are associated with neurodegenerative disorders such as Alzheimer’s and Huntington’s along with Parkinson’s and even simple senile dementia.  

GABA traps reactive intermediates (ROS) created during metabolism.  GABA has been able to capture the ROS created during lipid (cell membrane) peroxidation and to increase the activity of human antioxidant enzymes (superoxide dismutase and glutathione peroxidase).

The Benefits of NAC Supplementation

NAC is used in clinical settings to protect against systemic damage from harsh dyes used in imaging studies. It also protects from some chemotherapy agents. Likewise, it is an antidote for overdose from tylenol.

NAC acts to detoxify our bodies and brains. Glutathione is depleted by toxins that enter the system. NAC provides for restoration of normal levels

NAC supports our internal systems that protect us from stress, infections, inflammatory conditions and heavy oxidative stress; it does so through the antioxidant activities and by enhancing nitric oxide function.

Supplementing with NAC is known to increase levels of glutathione; this is body’s major naturally produced antioxidant.

Glutathione is composed of three amino acids. These are glutamate, glycine and cysteine. Of the three, cysteine is usually the limiting factor; it has the lowest intracellular levels. During stress stimulation (oxidative stress), limited amounts of cysteine halt production of glutathione. 

This results in excessive free radical formation and thus significant mitochondrial and cellular damage. NAC provides augmented levels of cysteine, helping to balance this process.

NAC And Neurodegenerative Diseases

For neurodegenerative disorders in particular, aging is thought to be the biggest risk factor. For example, Alzheimer’s disease (AD) generally affects only those over 65. The risk of AD doubles every 5 years after the age of 65.

There are difficulties with memory, problem-solving, cognition and memory.

One theory is that whatever causes aging must also cause neurodegeneration. Thus, there is blooming work in the field of anti-aging. Much of this research is centered on how antioxidants are linked to anti-aging benefits.

Genes and interventions that have been shown to enhance longevity in one species are conserved in all others. Also, lifespan-extending genes are protective of neurodegenerative disorders. 

Today, there is exciting work being done in the enhancement of molecular mechanisms of anti-aging.

What Is IN the multivWhy You Should Take An NAC Supplementitamin?

NAC does not require active transport and easily crosses cell membranes to be available for antioxidant production.

NAC reduces the activity of the NF-kB inflammation pathway. This hyperinflammatory pathway becomes especially active in times of stress and oxidative stress. NAC can suppress the damaging cytokine production that invariably results from the activation of the NF-kB path.

Many believe that neurodegenerative disorders are aging related proteinopathies. They are basically due to a build up of poorly folded proteins that are not repaired. These then lead to oxidative stress. This happens in a low GSH (glutathione) environment.

Many scientists believe that toxicity from bad protein in the brain could be cleaned up with the assistance of NAC, due to its abilities to stimulate production of glutathione.

Not only does NAC act as a free radical scavenger, but it is known to act as a life extender in mammal and invertebrate studies and for breast epithelial stem cells; clinical human findings of longevity have not been completed. 

How To Take NAC

Most oral NAC doses are at a 200-400mg range. Doses of 1200mg are discussed in some studies as well. Many advocate taking 600mg capsules up to three times a day.

All antioxidants have the potential to become pro-oxidant at certain doses, so finding supplements that contain appropriate levels of antioxidants is essential. However, in the setting of chronic inflammation and oxidative stress, this is less relevant. Doses of 1.2g daily would be entering the range of concern.
